About the Role

We are seeking a highly motivated and compassionate Case Manager to join our Site Based Housing Team at PATH (People Assisting the Homeless). As a key member of our team, you will play a vital role in assisting clients in breaking the cycle of homelessness.

Key Responsibilities
  • Conduct thorough intake and individualized needs assessments for all clients, collaborating to develop a personalized service plan that addresses housing goals, income maintenance, and other personal objectives.
  • Provide referrals, linkages, information, and support to resources that help clients achieve their service plan goals.
  • Complete program-specific assessments, such as the VI-SPDAT, to evaluate client strengths and challenges.
  • Evaluate client progress and conduct 90-day reviews to ensure effective service delivery.
  • Maintain accurate and up-to-date client files, adhering to program and PATH policies.
Requirements
  • Two years of experience in healthcare, case management, homeless services, or a related field.
  • Excellent analytical, communication, and problem-solving skills.
  • Ability to work effectively with diverse populations and maintain professional boundaries.
  • Valid driver's license and reliable transportation.
