Position Overview:

We are in search of a proactive and seasoned Senior Case Management Specialist to join our Mental Health team. This role is crucial in facilitating group, individual, and family counseling at our center, ensuring that the highest standards of care are upheld and innovative strategies are incorporated into our programs. The Senior Case Management Specialist will supervise Case Managers and manage a client caseload directly.

Key Responsibilities:

Standards, Policies, and Ethics:

  • Ensure adherence to The Forge Recovery Center's standards, policies, and ethical guidelines.
  • Safeguard client rights and uphold professional conduct.
  • Act decisively in crisis situations as required.

Clinical Expertise:

  • Exhibit proficiency in TAP21 Counselor Competencies.
  • Employ agency assessment tools in KIPU for thorough client evaluations.
  • Identify and prioritize client needs and concerns.

Client-Centered Care Planning:

  • Create personalized care plans centered on client-reported objectives.
  • Conduct therapy sessions utilizing evidence-based methodologies.
  • Document client progress through SOAP notes.
  • Prepare effective discharge plans and transition documentation.

Collaboration and Communication:

  • Support and mentor Case Managers to enhance their skills and knowledge.
  • Coordinate with health insurance providers for utilization reviews.
  • Engage in clinical treatment team meetings.
  • Provide accurate assessment and referral services.
  • Communicate effectively with probation officers, attorneys, and family members.

Professionalism and Compliance:

  • Adhere to ethical and legal standards.
  • Ensure timely and accurate clinical documentation.
  • Maintain awareness of cultural competency and client satisfaction.
  • Prioritize the safety of staff and clients.

Qualifications:

  • Case Manager Credential. RADT minimum, CADC I, SUDCC II, CATC II or higher preferred.
  • 3-5+ years of experience in Case Management.
  • Dedication to client-focused work and continuous improvement.
  • AA or BA degree in Human Services or Behavioral Health, or equivalent experience.
  • High proficiency in computer skills.

Additional Certifications & Licenses:

  • CPR/First Aid certification.
  • Valid Driver's License.

Other Requirements:

  • Negative TB test results within the past year.
  • Medical clearance to work within the past year.
  • Able to work evenings as necessary.
