We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Outreach Development. As a key member of our healthcare team, you will provide high-quality medical care and support to our clients in a residential substance use disorder treatment setting.
Job Summary:The LPN will work under the supervision of a Registered Nurse to provide various aspects of client medical care, including administering medication, monitoring health calls, and coordinating health services. You will work closely with clients, guardians, and healthcare professionals to ensure optimal health outcomes.
Responsibilities:- Administer and monitor medication calls and health calls as scheduled under the supervision of a Registered Nurse.
- Work with clients and guardians to schedule medical appointments and conduct follow-up as required.
- Collaborate with the Agency Psychiatrist regarding client medication and pharmacy refills.
- Provide on-site health services to address emerging and/or current health needs of clients.
- Conduct clinical procedures such as dressing changes and immunizations (with additional training).
- Obtain accurate medical data, including nursing intake/health history, tobacco use history, and communicable disease risk.
- Prepare new clients for admission physical examinations and report results of nursing care to the Primary Registered Nurse/Medical Director.
- Maintain adequate inventory of supplies and prepare exam rooms.
- Perform various office responsibilities, including managing inventory, breaking down inactive medical charts, and filing paperwork.
- NYS Licensed Practical Nurse (LPN) certification.
- Previous experience working as a Licensed Practical Nurse in a residential substance use disorder treatment setting preferred.
- Previous experience working in a related treatment, therapeutic community, community residence, and/or mental health setting working directly with special populations required.
This job operates in a professional office environment and outside in the field. You will be required to travel among assigned sites and locations as needed.
