Nutrition Care Specialist

Location: Bluestem PACE - McPherson, KS

Job Type: Full-time

Education Level: Bachelor's Degree in Nutrition or Dietetics

Experience: Experienced

Description:

This position involves assessing the nutritional status of PACE participants and developing tailored nutrition plans. The successful candidate will work closely with the interdisciplinary team to ensure that nutritional needs are met.

Key Responsibilities:

  • Conduct regular assessments for all participants
  • Collaborate with the team to develop care plans
  • Work with participants/caregivers to set individualized nutrition goals
  • Ensure continuity of care with a focus on positive outcomes
  • Effective communication with healthcare team members
  • Monitor participant health and report changes to the team
  • Oversee Culinary staff performance
  • Maintain adherence to relevant regulations and standards

Requirements:

  • Age requirement: 18 years
  • Kansas Registered Dietitian licensure
  • Clinical experience in medical nutrition therapy, particularly for elderly patients with chronic conditions
  • Able to instruct on safe food handling practices and kitchen equipment use

Physical Requirements: Light Work - Occasional exertion of up to 20 pounds
