Pediatric Hematology Oncology Nurse Navigator

Nemours Children's Hospital is seeking a skilled Pediatric Hematology Oncology Nurse Navigator to join our team in Orlando, Florida. As a Nurse Navigator, you will play a critical role in ensuring seamless care coordination for patients with complex needs.
Key Responsibilities:- Develop and enhance processes to improve clinical experience for referred patients and referring physicians.
- Coordinate multidisciplinary clinics, assisting with clinic flow and alerting providers to patient needs.
- Coordinate and delegate prior authorizations and peer-to-peer discussions.
- Participate in multi-disciplinary team discussions to develop individualized patient care plans.
- Facilitate and follow-up on lab tests, scans, and other diagnostic procedures.
- Evaluate patient and family response to care, tracking outcomes and relevant data using information systems and pathways.
- Promote and provide patient, family, staff, and community education using clinical expertise and knowledge base.
- Bachelor's Degree in Nursing required.
- Current Florida RN License in good standing.
- American Heart Association BLS Certification required.
- Minimum of one (1) year to three (3) years of experience required, with a minimum of two (2) years of clinical experience in pediatrics, HemOnc preferred.
- Obtain APHON certification within six (6) months of hire.
- CPHON, CPON, or BMCTN certification preferred.
