Job Title: LTSS Service Coordinator-RN Clinician

At Elevance Health, we are seeking a highly skilled LTSS Service Coordinator-RN Clinician to join our team. As a key member of our healthcare team, you will be responsible for managing the overall care of our members, ensuring they receive the best possible care and support.

Key Responsibilities:
  • Conduct thorough assessments of members' needs, identifying areas for improvement and developing personalized care plans to meet their unique requirements.
  • Collaborate with healthcare teams, including physicians, nurses, and other healthcare professionals, to ensure seamless care coordination and effective communication.
  • Develop and implement care plans that address members' physical, behavioral, and social health needs, with the goal of optimizing their overall health and well-being.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure the best possible outcomes for our members.
  • Provide education and support to members and their caregivers, empowering them to take an active role in their healthcare.
  • Interface with Medical Directors, Physician Advisors, and Inter-Disciplinary Teams to develop and implement care management treatment plans.
Requirements:
  • Current, unrestricted RN license in applicable state(s) required.
  • Minimum of 3 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role.
Preferred Skills and Experiences:
  • Proficiency in Microsoft Office (Excel, Word, Outlook, etc.) strongly preferred.
  • Case management experience strongly preferred.
  • Home health experience strongly preferred.
  • Medicaid experience strongly preferred.
  • Discharge planning experience strongly preferred.
  • MA/MS in Health/Nursing preferred.
