Compassionate Healthcare Practitioner for Hospice Services

We are seeking a compassionate and experienced RN Case Manager to join our team at Agrace. As a member of our interdisciplinary team, you will play a vital role in delivering patient-centered care and support. This is a full-time position offering a competitive salary of $85,000 per year, depending on experience.
About the PositionThe successful candidate will have one year of nursing experience, preferably in hospice or palliative care. A valid driver's license, auto insurance, and access to a reliable vehicle are required. The ideal candidate will be able to develop, maintain, and apply knowledge of hospice and end-of-life processes, while maintaining professional boundaries with patients, families, and colleagues.
Responsibilities- Patient and Family Assessments: Conduct thorough assessments of patients' physical, emotional, spiritual, and environmental needs.
- Patient Visits: Provide interventions, treatments, and pain management to meet patients' needs, while evaluating their effectiveness.
- Plan of Care: Develop, implement, and revise individualized plans of care in collaboration with the interdisciplinary team.
