Primary Healthcare Specialist

Bluestone Physician Services is seeking an experienced Primary Healthcare Specialist to provide high-quality medical care to patients in our Assisted Living and Memory Care communities.
About the RoleIn this position, you will work closely with our clinical support team to design and manage your schedule, develop and implement care plans, and make clinical decisions.
You will be responsible for providing on-site primary care services to patients, managing a panel of 200 patients, and collaborating with community partners.
Key Responsibilities- Provide primary care services to patients within Assisted Living and Memory Care communities
- Manage a panel of 200 patients, with approximately 12-16 patients per day
- Design and manage your own schedule
- Develop and implement care plans tailored to each patient's unique needs
- Collaborate with community partners, including home care, hospice, and other resources
To be successful in this role, you will need:
- A graduate degree from an accredited Nurse Practitioner Education Program or Physician Assistant Education Program
- Board certification through AANP, ANCC, or NCCPA
- Licensure through the Wisconsin Board of Nursing or Board of Medical Practice
- Ability to obtain DEA certification for the State of Wisconsin
- Excellent communication and interpersonal skills
