Care Manager RN Job Description

Children's Health (Dallas) is seeking a highly skilled Care Manager RN to join our team. As a Care Manager RN, you will be responsible for coordinating care throughout the continuum of care for an assigned patient population.

Key Responsibilities:

  • Prescribe, delegate, and coordinate patient care using clinical judgment and evidence-based guidelines.
  • Oversee care delivered by patient care team and coordinate plan of care.
  • Provide education and facilitate learning for patients, families, and patient care team.
  • Collaborate with physicians, families, and other healthcare professionals to develop and implement an appropriate plan of care.
  • Advocate for patients and represent their concerns to the healthcare team.

Requirements:

  • At least 4 years of pediatric nursing experience, case management, care management, care coordination, utilization review, or community-based nursing.
  • Two-year Associate's degree or equivalent experience required.
  • Four-year Bachelor's degree or equivalent experience preferred.
  • Registered Nurse in the State of Texas upon hire required.
  • Accredited Case Manager (ACM) or Certified Case Manager (CCM) or Care Coordination and Transition Management (CCTM) certification preferred.
