Children's Health (Dallas) is seeking a highly skilled Pediatric Registered Nurse Care Manager to join our team. As a key member of our care coordination team, you will play a vital role in ensuring the seamless transition of patients through the healthcare system.
Job Summary:The Pediatric Registered Nurse Care Manager will be responsible for coordinating care for pediatric patients across the continuum of care. This includes collaborating with healthcare providers, families, and other stakeholders to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities:- Coordinate care for pediatric patients across the continuum of care, including hospital, home, and community settings.
- Collaborate with healthcare providers, families, and other stakeholders to develop and implement individualized care plans.
- Assess patient needs and develop strategies to address gaps in care.
- Facilitate communication and coordination among healthcare providers, families, and other stakeholders to ensure seamless transitions of care.
- Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Current RN licensure in the State of Texas.
- At least 4 years of pediatric nursing experience, with a focus on care coordination and case management.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Ability to analyze complex patient data and develop effective care plans.
- Knowledge of pediatric healthcare systems and policies.
- Accredited Case Manager (ACM) or Certified Case Manager (CCM) certification.
- Experience with electronic health records and other healthcare technology systems.
- Knowledge of quality improvement principles and practices.
