As a key member of the Prisma Health team, you will play a vital role in ensuring the smooth flow of patients through our healthcare system. This position is responsible for coordinating patient admissions, transfers, and placements within our facilities, working closely with physicians, nurses, and other healthcare professionals to provide safe and timely care transitions.
Key Responsibilities- Standardize processes to ensure patients are admitted to the appropriate unit with the necessary orders.
- Develop and implement standardized processes for inpatient transfers and patient discharges.
- Evaluate bed turnaround times and develop action items to address opportunities for improvement.
- Collaborate with the Emergency Department, inpatient units, Administrators on Duty, and Environmental Services to improve care transitions.
To be successful in this role, you will need:
- A graduate degree in Nursing from an accredited program recognized by the SC State Board of Nursing.
- At least 3 years of experience in acute care nursing.
- A current RN license in South Carolina.
