Key Responsibilities:- Supervise and coordinate the care provided by LPNs, CNAs, and other team members
- Develop and implement individualized care plans for residents
- Assess and address the health needs of residents, notifying physicians of changes in status
- Provide safe and accurate medication-related interventions
- Contribute to the assessment and development of care plans
- 2+ years of experience in long-term care
- Additional certification in a nursing specialty desired
