Senior Care Coordinator with Clinical Reimbursement Expertise

About the RoleThe MDS Coordinator/Floor Nurse is responsible for overseeing the completion of the Minimum Data Set (MDS) and Resident Assessment Instrument (RAI) processes, ensuring compliance with federal regulations. This role involves collaborating with healthcare staff to manage resident assessments and care planning while also fulfilling duties as a floor nurse.
Key Responsibilities- Collaborate with DON, CRM, and various disciplines to complete the MDS, RAI, and associated processes.
- Ensure completion of assessments within mandated timeframes.
- Manage resident assessment processes and track Medicare/Managed Care/Medicaid case mix documents.
- Assist with resident care planning under the direction of the DON or CRM.
- Graduation from a School for Registered Nursing required.
- Minimum 1 year of previous experience in an MDS/Clinical Reimbursement Nurse role preferred.
- Current State Licensure as a Registered Nurse in the state where practicing is required.
- CPR and Automated External Defibrillator (AED) certifications required.
- Maintain a courteous and professional manner through interactions with others.
- Use a resident and customer-focused approach to problem solving and goal setting.
- Use discretion in handling confidential information.
- Excellent knowledge of case-Mix, and the Federal Medicare PPS process.
