Oncology Clinical Pharmacist

Position Overview
Role Summary: The Oncology Specialty Clinical Pharmacist will deliver specialized pharmaceutical care services within the oncology outpatient dispensary, adhering to established protocols in collaboration with healthcare teams. This role involves managing and dispensing oral and supportive oncology drug therapies, providing patient education, and ensuring the delivery of high-quality, evidence-based, and cost-effective pharmaceutical care. Independent judgment and decision-making are essential in this position.
Key Responsibilities:
- Facilitates rational, evidence-based, and cost-effective outpatient oncology drug therapy by actively participating in the decision-making process alongside the care team.
- Evaluates patient-specific drug therapy management plans under approved treatment protocols, recommending modifications based on comprehensive patient assessments.
- Enhances the utilization of oncology medication regimens to improve clinical outcomes across various cancer diagnoses.
- Assesses the appropriateness of medication administration, including dosage, route, and frequency, considering relevant clinical data.
- Understands specialty pharmacy benefit managers (PBMs) and formulary requirements, recommending alternatives for non-formulary specialty drugs.
- Secures prior authorizations for oncology medications through collaboration with PBMs and grant organizations.
- Documents and communicates findings and interventions with prescribers, providing alternatives to treatment plans as necessary.
- Promotes patient adherence to medication regimens through counseling and motivational interviewing techniques.
- Designs and monitors therapeutic plans for specialty drugs, ensuring improved patient outcomes through direct and telephonic interactions.
- Conducts inquiries into patient medication histories and provides drug information as needed.
- Implements systematic procedures for retrieving and analyzing drug information.
- Collaborates with healthcare professionals to evaluate the quality and effectiveness of drug regimens.
- Identifies and resolves medication-related issues, ensuring optimal therapeutic outcomes.
- Coordinates laboratory monitoring of drug therapy, interpreting results to guide treatment decisions.
- Evaluates the effectiveness of drug therapies and anticipates potential side effects.
- Participates in presentations for committees focused on quality improvement and pharmacy management.
- Engages in the analysis of oncology pharmacy data to identify opportunities for enhancing patient care.
- Contributes to operational and clinical projects as needed within the department.
- Participates in the development and delivery of educational initiatives for providers and staff.
- Ensures continuous improvement in specialty pharmaceutical care services.
- Provides insights on technology utilization for documenting clinical pharmacy outcomes.
- Undertakes additional duties as required.
- Pharm D degree required.
- PGY-1 Residency preferred.
- New Jersey State Board of Pharmacy licensure required.
- Board of Pharmaceutical Specialties Certification in Ambulatory Care or Pharmacotherapeutics required; other specialties considered.
- Certified Specialty Pharmacist credentials required.
- MTM Certification from the American Pharmaceutical Association preferred.
- Proficient communication skills in English, both written and verbal.
- Experience with standard office equipment and technology in a Windows environment.
- Preferred experience in specialty clinical pharmacy, particularly in retail or PBM settings, with a minimum of 5 years.
- Adherence to local, state, and federal pharmacy practice regulations.
