Oncology Clinical Pharmacist

Position Overview
Role Summary: The Oncology Specialty Clinical Pharmacist will deliver specialized pharmaceutical care services within the outpatient dispensary, adhering to established protocols and working in conjunction with a multidisciplinary healthcare team. This role involves managing and dispensing oral oncology medications and supportive therapies while providing comprehensive patient education to ensure high-quality, evidence-based, and cost-effective pharmaceutical care. Independent judgment and decision-making are essential in this position.
Key Responsibilities:
- Facilitates the implementation of evidence-based and economically viable outpatient oncology drug therapies through active participation in treatment decision-making with the healthcare team.
- Evaluates patient-specific medication management plans under approved treatment protocols, recommending modifications based on thorough assessments and clinical data.
- Enhances the utilization of oncology medications and improves clinical outcomes across various cancer types.
- Assesses the appropriateness of medication regimens, including administration routes, dosages, and frequencies, while considering patient-specific factors.
- Understands and navigates Specialty Pharmacy Benefit Managers (PBMs) and formulary guidelines, suggesting alternative therapies when necessary.
- Secures prior authorizations for oncology medications by collaborating with PBMs and assistance programs.
- Documents and communicates clinical findings and interventions to prescribers, offering alternative treatment options as required.
- Promotes patient adherence to medication regimens through counseling and motivational interviewing techniques, documenting consultations and therapy progress.
- Designs and monitors therapeutic plans for specialty medications, engaging with patients and providers through direct and telephonic interactions.
- Conducts comprehensive medication history assessments and responds to drug information inquiries from healthcare professionals.
- Employs systematic methods for retrieving and analyzing drug information, ensuring accurate and timely responses.
- Collaborates with healthcare professionals to evaluate and enhance the quality and effectiveness of drug therapies.
- Identifies and mitigates potential medication-related issues, ensuring optimal patient safety and therapeutic outcomes.
- Coordinates laboratory monitoring related to drug therapy, interpreting results to inform clinical decisions.
- Participates in quality improvement initiatives and presents findings to relevant committees.
- Engages in operational and clinical projects aligned with departmental goals.
- Contributes to the development and delivery of educational resources for healthcare providers and staff.
- Ensures continuous improvement in specialty pharmaceutical care services.
- Provides insights on technology utilization for documenting clinical outcomes and addressing medication-related challenges.
- Performs additional duties as assigned.
- Doctor of Pharmacy (Pharm D) degree required.
- Completion of a PGY-1 residency preferred.
- Current NJ State Board of Pharmacy licensure required.
- Board Certification in Ambulatory Care or Pharmacotherapy preferred; other specialties considered.
- Certification as a Specialty Pharmacist required.
- MTM Certification from the American Pharmaceutical Association preferred.
- Proficient in English communication, both verbal and written.
- Familiarity with standard office equipment and technology in a Windows environment.
- 5+ years of experience in Specialty Clinical Pharmacy preferred.
- Adherence to all relevant pharmacy practice regulations.
