Job Title: Clinical Care Partner

About the Role

We are seeking a skilled Clinical Care Partner to join our team at Franciscan Missionaries of Our Lady Health System. As a Clinical Care Partner, you will play a vital role in coordinating pre-visit patient care, ensuring that our patients receive the highest level of service and care.

Key Responsibilities

  • Monitor disease registry for patient scheduled appointments, procedures, tests, and referrals
  • Communicate high-risk patient needs to registered nurse care coordinators
  • Relies on guidelines to accomplish tasks and works under close supervision

Requirements

  • 3 years of experience as a medical assistant
  • Graduate of an accredited medical assistant program or 2 years of additional medical assistant experience in lieu of education
  • Proficient in the English language, verbal and written communication skills, and computer skills
  • BLS Certification
