Advanced Practice Clinician

We are seeking a highly skilled and compassionate Advanced Practice Clinician to join our team at Summit Health, Inc.
About the Role:
As a Nurse Practitioner/Physician Assistant, you will be responsible for providing high-quality, patient-centered care to adult patients in a fast-paced, dynamic environment.
Key Responsibilities:
- Conduct comprehensive health histories and physical assessments of patients with acute, chronic, and potential health problems.
- Develop and implement individualized treatment plans to meet the unique needs of each patient.
- Prescribe medications and order diagnostic tests as necessary.
- Collaborate with other healthcare professionals to ensure seamless care coordination.
- Stay up-to-date with the latest medical research and best practices to provide evidence-based care.
Requirements:
- Graduate of an accredited advanced practice program.
- Active DEA registration or eligibility for registration.
- Active Medicare/Medicaid enrollment or eligibility to enroll.
- Comfortable with Electronic Medical Record (EMR) systems and Microsoft Office.
