Position Summary
Under general supervision, you will be responsible for diagnosing and managing patient care in collaboration with physician colleagues. This role emphasizes continuity of care by facilitating communication among the healthcare team, patients, and their families. You will coordinate care to ensure quality, cost-effective practices, appropriate length of stay, and patient safety, while integrating relevant research and best practices.
Key Responsibilities

Clinical Duties
- Collaborate with attending physicians to provide clinical direction and leadership for the healthcare team.
- Conduct comprehensive assessments for patients admitted or scheduled for procedures.
- Order and interpret necessary diagnostic tests.
- Establish medical diagnoses based on thorough evaluations.
- Develop and document treatment plans, including medication prescriptions based on detailed assessments.
- Initiate clinical protocols and evaluate care plans for timely interventions.
- Perform necessary invasive procedures.
- Work with attending physicians and residents on admission, discharge, and transfer decisions.
- Present patient cases during rounds.
- Communicate care plans effectively with healthcare providers, patients, and families.
- Provide education and counseling to patients and families regarding treatment plans and health maintenance.
- Facilitate appropriate referrals and consultations as needed.
- Ensure smooth patient flow and collaborate with multidisciplinary teams for timely discharges.
- Advocate for patients, ensuring their participation in care decisions and understanding of treatment options.
- Consider holistic patient needs and coordinate necessary resources.
- Perform additional activities as outlined in individual agreements.
Educational Responsibilities
- Provide educational programs for healthcare team members based on evidence-based practices.
- Participate in mentoring and precepting students.
- Enhance understanding of disease processes within the specialty.
- Act as a clinical resource for team members, including interns and medical students.
- Engage in community education and health promotion activities.
Leadership and Outcomes
- Coordinate nurse practitioner coverage within the service.
- Collaborate with physician leaders to achieve service-specific outcome targets.
- Participate in performance improvement initiatives aimed at enhancing clinical outcomes.
- Contribute to the development of clinical pathways in collaboration with the multidisciplinary team.
- Lead quality assurance activities and establish standards of practice.
