Job Title: Case Manager

We are seeking a highly skilled and compassionate Case Manager to join our team at The Salvation Army USA Central Territory. As a Case Manager, you will play a critical role in providing supportive services to Veteran households, helping them achieve housing stability and overcome challenges related to homelessness.

Key Responsibilities:
  • Provide case management interventions to Veteran households, including those with high acuity needs, mental illness, substance abuse, and other barriers to housing stability.
  • Develop and implement housing stability plans, addressing crisis needs, housing barriers, and participant goals.
  • Conduct outreach, engagement, and community networking to identify and connect Veteran households with available resources and services.
  • Screen Veteran households for eligibility and provide referrals for those not eligible for our services.
  • Document progress, linkages, and referrals to permanent housing, mainstream, Veteran-serving, and community-based resources.
Requirements:
  • Bachelor's degree in social work or a related field.
  • 2-5 years of relevant experience, preferably in case management, outreach, and engagement with high-need populations.
  • Strong communication and interpersonal skills, with the ability to build trust and rapport with Veteran households.
  • Proficiency in motivational interviewing, crisis intervention, and trauma-informed approaches.
  • Ability to operate technology, including personal computers, copiers, and scanning equipment.
