Job Title: Case Manager

Join The Salvation Army USA Central Territory as a Case Manager and make a meaningful impact in the lives of veterans and their families. As a key member of our team, you will provide case management and supportive services to eligible veterans, helping them achieve housing stability and overcome challenges.

Key Responsibilities:
  • Provide case management and supportive services to eligible veterans, including delivery of Rapid Resolution, Rapid Rehousing, and Homeless Prevention services.
  • Conduct outreach, engagement, and community networking to identify and develop relationships with potential participants, referral sources, and community partners.
  • Assist with staff training and onboarding, providing job shadowing and specific training related to case management interventions and case manager skill development.
  • Screen veteran households for eligibility and provide referrals for those not eligible.
  • Conduct intake and assessment, including documentation of veteran status, household income, referral needs, housing barriers, mental/health needs, and safety concerns.
  • Develop and maintain housing stability plans, addressing crisis needs, housing barriers, and obtaining and maintaining permanent housing.
  • Provide advocacy and mediation services, including court appointments, landlord-tenant negotiation, conflict resolution, and payment of rent or utility arrears.
  • Coordinate case management services with the Veterans Affairs (VA), Continuum of Care (CoC), mainstream, community-based, and legal resources.
  • Maintain accurate and comprehensive case file documentation, including prioritization list management, ongoing assessment, progress, accomplishments, challenges, barriers, housing stability, eligibility re-certification, discharge planning, and follow-up.
Requirements:
  • Bachelor's degree in social work or related field.
  • 2-5 years of relevant experience, preferably with motivational interviewing, solution-focused interventions, and mediation serving high-need populations.
  • Proficiency in Motivational Interviewing, Crisis Intervention, and/or Trauma-Informed approaches.
  • Ability to operate telephone/mobile phone and electronic communication devices, as well as other office equipment.
  • Valid driver's license and approval through The Salvation Army Fleet Safety Program.
