Job Summary

We are seeking a skilled Home Health Physical Therapist Manager to join our team at AdventHealth. As a Physical Therapist Care Manager, you will be responsible for coordinating and directing the care of a caseload of home patients requiring physical therapy as the primary skill.

Your primary focus will be on providing comprehensive assessment, planning, implementation, and evaluation for your caseload as the primary Care Manager. You will set priorities of home care caseload adapting to the changing needs of the home care patients and families, optimizing your schedule daily to support productivity, efficiency, and maintain best practice visit utilization.

You will perform evaluations, assessing function using a method which objectively measures activities of daily living, documenting the measurement results in the clinical record. You will utilize assessment data to formulate a patient-specific plan of care along with the patient, family, and physician, establishing individualized, realistic, measurable patient-centered goals in consultation with the patient, family, and other healthcare providers.

You will periodically reassess the patient every 30 days, providing the ordered therapy service, functionally reassessing the patient, and comparing the resultant measurement to prior assessment measurements. You will document the measurements in the clinical record along with your determination of the effectiveness or therapy or lack thereof.

You will instruct the patient and caregiver in care and use of assistive devices and the activity modifications, establishing a home program. You will instruct other health team personnel, including when appropriate, home health aides and caregivers, in certain phases of therapy with which they may work with the patient.

You will select, apply, or modify skilled intervention consistent with your training and scope of practice using various procedures and techniques to achieve the best outcome possible for the patient. You will assess patient response to interventions and perform reassessments as required.

Requirements
  • Minimum of one-year relevant clinical physical therapy experience
  • Master's degree in Physical Therapy
  • Recent, relevant experience in a Medicare-certified home health agency as a case-manager
  • Current Physical Therapy License in State of Practice
  • Valid Driver's License and current car insurance
  • CPR certified
  • Home Health Case-Manager Certification
