We are seeking a skilled Physical Therapist to join our Home Health team as a Care Manager. As a key member of our team, you will be responsible for coordinating and directing the care of our patients in their homes.
Key Responsibilities- Conduct comprehensive assessments of patients to identify their needs and develop personalized care plans.
- Collaborate with patients, families, and healthcare providers to ensure seamless care coordination.
- Provide education and training to patients and caregivers on safe and effective care practices.
- Monitor patient progress and adjust care plans as needed to achieve optimal outcomes.
- Identify opportunities for performance improvement and contribute to the development of initiatives to enhance patient care and outcomes.
- Minimum of one year of experience as a Physical Therapist in a home health setting.
- Active Florida Physical Therapist license.
- Basic Life Support (BLS) certification.
- Driver's license.
