Camden Case Management is seeking a dedicated Registered Nurse with specialized training in mental health to join our team as a PMHN Case Manager.

Job Summary:

The PMHN Case Manager will provide essential support and resources to our clients as they undergo treatments and procedures prescribed by their medical providers. This role requires a unique blend of skills and experience, including coordination with medical teams, arranging transportation, monitoring clients during treatments, accurately documenting interactions and progress, and providing medication management support as required.

Key Responsibilities:

  • Coordinate with clients' medical providers to schedule and manage treatments, ensuring effective care coordination.
  • Offer transportation assistance to clients, facilitating their transportation to and from scheduled treatments.
  • Provide attentive support and monitoring to clients throughout their treatment days.
  • May assist clients with medication counts and adherence, as needed.
  • May include both face-to-face and remote support as necessary on treatment days.
  • Maintain accurate documentation of all client interactions and treatment progress.

Requirements:

Familiarity with mental health treatment and services is preferred. Some trauma and addiction informed skills are also preferred. Open to and encouraged to be pursuing education in the field of mental health/counseling/addiction/coaching studies. CPR certified. Valid driver's license and clean driving record. Strong written and verbal communication skills and ability to manage potential issues arising with mentor, client, family members, sources of financial support and outside clinical providers. Maintain a flexible schedule including weekends and holidays (all work can occur remotely except client sessions which must occur in Camden Case Management offices). Able to remain empathetic, professional, and communicate effectively in stressful situations or crises.
