About the Role

We are seeking a skilled Registered Nurse to join our team as a Care Manager AIM (Advanced Illness Management) Full Time RN. This role will report to the Director of Care Management AIM and will be responsible for working with patients, their families, PCP, IDT, and other healthcare professionals to provide and coordinate high-quality end-of-life care.

Responsibilities
  • Develop and maintain rapport with patients and families, and work well with an interdisciplinary team
  • Provide direct patient care in homes and facilities
  • Collaborate with healthcare professionals to provide comprehensive care
  • Coordinate care plans and ensure seamless transitions
Requirements
  • 2 years of prior skilled Registered Nurse experience
  • Active acute unencumbered state RN license
  • Ability and interest to provide direct patient care in homes and facilities
  • Demonstrated experience developing and maintaining rapport with patients and families
