In this critical role, you will be responsible for scheduling visits and communicating with field staff, patients, physicians, and other stakeholders to ensure seamless care coordination and continuity of care.

Key Responsibilities:
  • Manage patient schedules, ensuring timely and efficient care delivery.
  • Communicate effectively with field staff, patients, and physicians to address any concerns or issues.
  • Assist with day-to-day office and staff management, including scheduling and coordination.
  • Initiate infection control forms as needed and maintain accurate records.
  • Provide exceptional customer service and support to patients and families.
Requirements:
  • Current LPN licensure in the state of employment.
  • At least 1 year of home health experience.
  • Prior packet review/QI experience preferred.
  • Coding certification is a plus.
  • Valid state driver's license and automobile liability insurance.
