Patient Services Coordinator-LPN

Key Responsibilities:- Manage patient schedules, ensuring timely and efficient care delivery.
- Communicate effectively with field staff, patients, physicians, and other stakeholders to maintain proper care coordination.
- Assist with day-to-day office and staff management, including scheduling, reporting, and data entry.
- Initiate infection control forms as needed and maintain accurate records.
- Provide exceptional customer service, responding to patient and staff inquiries in a timely and professional manner.
- Collaborate with the Branch Director to ensure smooth operations and address any concerns or issues.
- Current LPN licensure in the state of employment.
- At least 1 year of home health experience, with a focus on care coordination and scheduling.
- Prior experience with packet review and QI is preferred.
- Coding certification is a plus.
- Valid state driver's license and automobile liability insurance.
- Excellent communication and organizational skills, with the ability to manage multiple assignments and prioritize tasks effectively.
