Position Overview:
The RN Case Manager is responsible for planning, coordinating, and delivering hospice care in alignment with the patient's individualized care plan.
Key Responsibilities:
- Collaborate with the interdisciplinary group (IDG) and family members to ensure comprehensive care delivery.
- Continuously assess the effectiveness of hospice services for patients and their families.
- Conduct evaluations and informational visits as necessary.
- Utilize the nursing process to provide high-quality nursing care.
- Consult with physicians regarding any changes in patient conditions that may require adjustments to care plans.
- Coordinate services with other IDG members and external contracted services as needed.
- Document clinical notes accurately and in a timely manner.
- Participate in IDG meetings and case conferences to discuss patient care.
- Maintain up-to-date patient care plans and ensure compliance with regulations.
- Perform on-call duties as required.
- Engage in quality assurance and performance improvement activities.
- Provide education to patients and families regarding care plans and the dying process.
- Conduct comprehensive assessments of patients, including medical history and physical examinations.
- Supervise and evaluate the care provided by Hospice Aides.
Job Conditions:
This position may involve stressful situations and requires the ability to access various patient environments. Physical demands include walking, standing, and lifting patients as necessary.
Qualifications:
- Graduate from an accredited School of Nursing.
- Current RN license in good standing.
- Minimum of one year of nursing experience, preferably in hospice care.
- CPR certification.
- Strong assessment and problem-solving skills.
- Excellent communication and organizational abilities.
