Healthcare Coordinator

Job Summary:
The Community Nurse is a critical member of our healthcare team, responsible for coordinating and delivering high-quality care services to our residents. This role requires a strong clinical background, excellent communication skills, and the ability to work collaboratively with our staff to meet the unique needs of each resident.
Key Responsibilities:
- Coordinate and oversee the delivery of healthcare services to residents, ensuring timely and effective care.
- Participate in individual service planning, including the development and implementation of care plans.
- Recommend and implement health care service goals, working closely with residents, families, and healthcare providers.
- Supervise and coordinate the work of House Managers and caregivers to ensure resident needs are met.
- Provide clinical services as needed to residents, including training and education for staff.
- Ensure compliance with state regulations and the Nurse Practice Act, including delegation of medication administration and health care tasks.
Requirements:
To be successful in this role, you will need:
- A strong clinical background, with a focus on community health and nursing practice.
- Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare providers.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
