Job Description

The MDS Coordinator will be responsible for coordinating the care planning process, conducting assessments, and developing individualized care plans in collaboration with the interdisciplinary team. You will also be responsible for monitoring resident progress, identifying areas for improvement, and implementing changes as needed.

Requirements
  • Bachelor's degree in Nursing (BSN) or related field
  • Current RN licensure and CPR certification
  • Minimum 1 year of experience in long-term care or related field
Compensation and Benefits
