Job Summary

We are seeking a highly motivated and compassionate Patient Navigator/Driver to join our team at Gateway Community Health Center, Inc. As a key member of our interdisciplinary team, you will play a vital role in providing navigation services to address the social determinants of health for patients enrolled in our Community Mental Health Grant Program.

Key Responsibilities:

  • Work closely with the interdisciplinary team to assess and recognize the necessary treatment and management for patients.
  • Develop and maintain case files for each patient, identifying primary care and mental services needed, and facilitating referrals as necessary.
  • Provide peer support group services, health education, skills development, and medication management to patients.
  • Ensure a smooth transition for patients from one healthcare provider or setting to another, minimizing fragmentation in their medical care.
  • Evaluate patients using the Patient Health Questionnaire-8 for children and Patient Health Questionnaire-9 for adults, and assess social determinants of health using the PRAPARE Assessment.
  • Maintain confidentiality of patient information per HIPAA guidelines.

Requirements:

  • Bachelor's degree in Social Work, Sociology, Psychology, Health Education, or a related field from an accredited college or university.
  • College education may be substituted for three years' experience in a healthcare setting.
  • Ability to work long hours during the week and weekends as necessary.
  • Bilingual in English and Spanish is preferred.
  • Possess means of transportation and a valid Texas Driver's License with minimum liability insurance.
