Overview

Segue Health Management Corp is a faith-based medical practice seeking an experienced Nurse Practitioner to join our team. This is a full-time opportunity for a qualified professional to provide high-quality care to patients in Gibson, Weakley, and Obion counties.

About the Role

We are looking for a skilled Nurse Practitioner who can work independently and collaboratively as part of a healthcare team. The ideal candidate will have excellent communication skills, be able to travel up to 1 hour away from home, and possess a current medical license to practice within the state of TN.

Responsibilities
  • Provide medical consults to patients in their homes after discharge from an acute care facility
  • Review discharge summaries and progress notes to ensure continuity of care
  • Collaborate with home health teams and other healthcare professionals to develop effective treatment plans
  • Prescribe medications when necessary and manage patient medication regimens
Requirements
  • Current medical license to practice within the state of TN
  • Current Medicare number
  • Ability to travel up to 1 hour away from home
