Job Description:
We are seeking a highly skilled Primary Care Provider to join our team of dedicated professionals in long-term care facilities. As a Primary Care Provider, you will play a vital role in delivering comprehensive care to patients, working closely with a multidisciplinary team to ensure seamless coordination of care.
Your Responsibilities Will Include:
- Providing high-quality patient care, including assessment, diagnosis, and treatment planning
- Conducting thorough medical histories and physical examinations
- Ordering and interpreting diagnostic tests
- Developing and implementing individualized care plans
- Prescribing medications and administering treatments as needed
- Collaborating with the interdisciplinary team to ensure continuity of care
- Managing chronic conditions and monitoring patient progress
Requirements:
To be considered for this rewarding opportunity, you must possess:
- A Nurse Practitioner License
- A DEA License
- At least 6 months of experience in long-term care
