Job Description:

We are seeking a highly skilled Primary Care Provider to join our team of dedicated professionals in long-term care facilities. As a Primary Care Provider, you will play a vital role in delivering comprehensive care to patients, working closely with a multidisciplinary team to ensure seamless coordination of care.

Your Responsibilities Will Include:

  • Providing high-quality patient care, including assessment, diagnosis, and treatment planning
  • Conducting thorough medical histories and physical examinations
  • Ordering and interpreting diagnostic tests
  • Developing and implementing individualized care plans
  • Prescribing medications and administering treatments as needed
  • Collaborating with the interdisciplinary team to ensure continuity of care
  • Managing chronic conditions and monitoring patient progress

Requirements:

To be considered for this rewarding opportunity, you must possess:

  • A Nurse Practitioner License
  • A DEA License
  • At least 6 months of experience in long-term care
