Job Summary

We are seeking a skilled Behavioral Healthcare Coordinator to provide care coordination to members with behavioral health conditions. This role requires a licensed professional with 5 years of post-master's experience, able to navigate multiple systems and provide intensive interventions and oversight.

Key Responsibilities
  • Provide care coordination to members with behavioral health conditions, promoting the appropriate use of clinical and financial resources.
  • Collaborate with multiple clinical, social, and community resources to ensure comprehensive care.
  • Develop and implement individualized care plans to improve quality of care and member satisfaction.
  • Work closely with employers to understand their needs and provide tailored solutions.
Requirements
  • Master's degree in a related field (e.g., social work, counseling, psychology).
  • 5 years of post-master's experience in behavioral health care coordination or a related field.
  • Licensed as a LCSW-C, LCPC, or LCMFT.
  • Ability to work in a cubicle environment, type, and talk simultaneously at a conversational pace.
  • Flexibility to work evening shifts and adapt to changing schedules.
