Job Summary

This Home Health and Rehabilitation Support Specialist position involves providing critical support to individuals with brain injuries, assisting with daily living activities, rehabilitation, and social reintegration. The successful candidate will work collaboratively with our multidisciplinary team to develop and implement individualized care plans, promoting client autonomy and well-being.

Responsibilities:
  • Personal Care and Daily Activities: Assist clients with personal care, household management, and participation in community activities, ensuring their physical and emotional needs are met.
  • Rehabilitation and Life Skills Training: Support clients in developing essential life skills to foster independence, including nutrition planning, meal preparation, and time management.
  • Care Planning and Coordination: Collaborate with healthcare professionals to create and implement comprehensive care plans, addressing medical, emotional, and social needs.
  • Companionship and Emotional Support: Provide emotional support and companionship throughout the rehabilitation process, promoting a sense of connection and belonging.
  • Progress Monitoring and Adjustment: Regularly monitor and track client progress, making adjustments to care plans as necessary to ensure optimal outcomes.
Requirements:
  • Education: High school diploma or equivalent required; relevant experience or certifications in brain injury care, home health, or personal care preferred.
  • Communication and Interpersonal Skills: Strong communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare teams.
  • Flexibility and Adaptability: Ability to adapt to changing situations, priorities, and client needs, demonstrating flexibility and a willingness to learn.
  • Transportation and Mobility: Valid driver's license and reliable transportation, with the ability to travel to various locations as needed.
Compensation and Benefits

We offer a competitive salary range of $34,000 - $41,600 per year, based on location and experience. In addition, our benefits package includes opportunities for professional growth and development, paid time off, and a supportive work environment that prioritizes employee well-being.
