Job Overview

We are seeking a skilled and compassionate Home Health Aide to join our team. As a Home Health Aide, you will play a vital role in providing high-quality patient care and supporting the rehabilitation and recovery of our patients in the comfort of their own homes.

Responsibilities
  • Provide direct patient care under the supervision of a Registered Nurse (RN) and according to the individualized Plan of Care (POC).
  • Assist patients with self-administered medications, ensuring accurate dosing and adherence to prescribed treatment plans.
  • Take temperature, pulse, and blood pressure measurements, recognizing and reporting any abnormal results to the RN or supervisor.
  • Support patients with daily living activities, including bathing, grooming, skin care, shaving, oral care, nail/foot care, and other essential tasks.
  • Assist with feeding patients, promoting healthy nutrition and hydration practices.
  • Help patients maintain mobility and independence through safe transfers, positioning, and ambulation, as per the Plan of Care and CenterWell Home Health policy.
  • Document interactions with patients, caregivers, doctors, and other staff members, ensuring continuity of care and compliance with regulatory requirements.
  • Practice infection control principles, maintaining a clean and safe environment for patients and staff.
Requirements
  • High school diploma or equivalent required.
  • Completion of Certified Nursing Assistant (CNA) or Certified Home Health Aide (CHHA) program within the last 24 months.
  • Meet applicable state certification requirements.
  • A valid driver's license, auto insurance, and reliable transportation are necessary for travel between patient locations.
  • Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare-certified agency.
  • At least one year of experience as a Home Health Aide or CNA in a hospital, nursing home, home health/hospice agency within the last 24 months.
