We're seeking an Advanced Practice Clinician - Home Visits to join our interdisciplinary team in Olympia. As a key member of our Home-based Medical Care team, you'll provide exceptional patient care while working collaboratively with PCPs, specialists, and other healthcare professionals.

Responsibilities include managing chronic illnesses, documenting timely, and engaging new patients. You'll also collaborate with patient's PCP and specialists as needed. With a strong focus on wellness, avoiding unnecessary hospitalizations and ER visits, and improving overall quality of care, you'll make a meaningful impact on patients' lives.

To succeed in this role, you'll need certification through the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC), current NP licensure in Washington state, and access to reliable transportation. A proven knowledge of CPT, ICD-10, and HCPC codes is a plus.
