SalaryThe estimated salary for this role is $88,000 to $173,200 annually, based on factors such as local labor markets, education, work experience, certifications, and location. Job DescriptionIn this role, you will conduct in-home assessments, including past medical history reviews, medication reconciliation, vital signs checks, and comprehensive physical exams. You will also perform evidence-based practice screenings and identify diagnoses for care management and active medical management. Your primary responsibilities include:
- Conducting in-depth patient evaluations, analyzing medical histories, and providing personalized care plans.
- Developing treatment plans, coordinating follow-up care, and improving overall patient outcomes.
- Collaborating with healthcare teams to ensure seamless care transitions and effective communication.
You'll have the opportunity to achieve work-life balance with flexible scheduling, no on-call or weekend shifts required, and all travel expenses covered. Approximately 85% of your time will be spent traveling across Washington state, covering various dwelling conditions and navigating stairs.
Required Skills and QualificationsTo succeed in this role, you must possess the following qualifications:
- An active NP or PA license or ability to obtain by start date, with unencumbered licensure.
- A national NP or PA certification, such as ANCC or NCCPA, or the ability to obtain it by start date.
- The ability to complete the job's physical requirements, including lifting up to 30 pounds, navigating stairs, and performing physical assessments.
- Access to reliable transportation for up to 85% of your time within licensed states.
