Job Summary

We are seeking a highly skilled and motivated Pharmacist to join our team at PharMerica. As a Clinical Pharmacy Specialist, you will play a critical role in providing exceptional patient care and support to our long-term care facilities.

Key Responsibilities:

  • Evaluate physician medication orders for appropriateness and make recommendations for therapy changes as needed.
  • Maintain accurate and up-to-date patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medication.
  • Collaborate with the healthcare team to provide clinical and technical support as needed.
  • Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals.
  • Assist facility/agency staff personnel with procedures relating to safe and effective methods of procuring, maintaining, administering, and disposing of drugs.

Requirements:

  • Bachelor's Degree in Pharmacy; Pharm D preferred.
  • One year experience in long-term care.
  • Detail-oriented, goal-oriented, team player, and excellent oral communication skills.
