Pharmacist Position

We are seeking a highly skilled and motivated pharmacist to join our team at PharMerica. As a pharmacist, you will play a critical role in providing exceptional patient care and ensuring the safe and effective use of medications.
Responsibilities:- Evaluate physician medication orders for appropriateness, potential interactions, and route of administration.
- Maintain patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medication.
- Ensure laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with the healthcare team.
- Collaborate with the healthcare team to provide clinical and technical support as needed.
- Document all communication with physicians, nurses, and other healthcare team members in the medical record.
- Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals.
- Obtain and maintain required documentation in accordance with federal and state regulatory agencies and policy.
- Inform the Pharmacy Manager of medication-related errors.
- Assist facility/agency staff with procedures relating to safe and effective methods of procuring, maintaining, administering, and disposing of drugs.
- Bachelor's Degree in Pharmacy; Pharm D preferred.
- One year experience in long-term care.
