- Bachelor's degree in Occupational Therapy and five or more years of experience as an occupational therapist, OR Master's in Occupational Therapy and one or more years of experience as an occupational therapist
- Current valid State of Florida license as an Occupational Therapist
- Basic Life Support (BLS) Certification
