Job Summary

We are seeking a skilled Physician to join our Palliative Care Consultation Team and Hospice Interdisciplinary Team. As a key member of our team, you will be responsible for directing the medical component of care for assigned patients, participating in the development and implementation of patient care plans, and providing pain and symptom management consultation in various settings.

Key Responsibilities
  • Participate in the development and implementation of patient care plans, collaborating with the Palliative Care Team and other healthcare professionals.
  • Direct patient medical care and treatment, consulting and cooperating with attending/primary physicians as needed.
  • Provide pain and symptom management consultation in patient homes, assisted living facilities, hospitals, nursing homes, clinics, and Care Dimensions' Hospice Houses.
  • Assume the role of attending/primary physician as appropriate, ensuring continuity of patient care.
  • Function as an active consultant to area providers and facilities, providing clinical expertise on diagnosis and medical management of patient needs.
  • Educate and serve as a liaison between Care Dimensions, community physicians, and other healthcare providers to develop and expand outreach to the professional community.
  • Participate in Care Dimensions Speakers Bureau, as appropriate, and make regular contact with practicing physicians to introduce the Hospice and Palliative Care Program, encourage utilization, and answer clinical and other concerns.
  • Develop clinical hospice and palliative care standards of practice and protocols to ensure continuity of patient care delivery.
  • Prepare written reports as required by state and federal regulations and the needs of the agency.
  • Provide physician consultation services for mutually agreed-upon long-term or home care Care Dimensions team.
  • Participate in patient care planning by attending IDT meetings and reviewing medical information on Care Dimensions patients.
  • Determine patient eligibility for Hospice services in accordance with program admission criteria.
  • Certify patients' terminal illness and prognosis, and recertify patients' eligibility for Hospice services at intervals specified by CMS.
  • Assume responsibility for the general medical needs of Care Dimensions patients, as needed.
Requirements
  • Current license to practice medicine in the Commonwealth of Massachusetts.
  • Board certification in Internal Medicine, Family Practice, or a related field, with a minimum of two years' experience.
  • Privileges or eligibility for privileges as an attending physician with one or more Hospice of the North Shore contracted acute care hospitals.
  • Successful completion of a palliative care fellowship program, strongly preferred.
  • Eligibility and/or certification by the American Academy of Hospice and Palliative Medicine (AAHPM), desirable; willingness to obtain certification within two years required.
  • Demonstrated competency in symptom control measures and appropriate palliative care for terminally ill patients.
  • Demonstrated understanding of and support for an IDT approach to patient care planning and delivery.
  • Excellent verbal and written communication skills, with individuals and groups.
  • Strong interpersonal skills, with the ability to function as a member of an interdisciplinary team.
