Palliative Care Social Worker

Care Dimensions is seeking a compassionate and dedicated Social Worker to join our Palliative Care and Primary Care teams. As a Geriatrics and Palliative Care Social Worker, you will play a critical role in supporting older patients with life-limiting, complex illnesses, as well as those who have difficulty leaving their homes.
Key Responsibilities- Implement care plans to provide psychosocial assessment and support
- Care navigation and coordination of care
- Facilitate discussions to clarify goals of care and advance care planning
- Address bridge to hospice care when indicated
- Master's degree in Social Work or a related field
- Current Massachusetts social work licensure (LICSW preferred, LCSW accepted with relevant experience)
- Minimum of three years of counseling experience
- Experience working with patients with life-limiting, complex illnesses and their families
- Knowledge of the healthcare delivery system, community resources, hospice, and home care services
- Experience in geriatrics and palliative care preferred
