About the Role

We are seeking a skilled Care Manager - Behavioral Health Specialist to join our team at Superior HealthPlan. As a key member of our care management team, you will play a critical role in developing and implementing holistic care plans for our members with complex mental and behavioral health needs.

Key Responsibilities
  • Evaluate member service needs and develop or contribute to the development of care plans/service plans to ensure quality, cost-effective healthcare outcomes.
  • Act as a liaison and member advocate between the member/family, physician, and facilities/agencies to ensure seamless coordination of care.
  • Support members with primarily mental/behavioral health needs, such as those with (or a history of) major depression, bipolar disorders, schizophrenia, borderline personality disorder, post-traumatic stress disorder, substance use disorder, self-injurious behavior, psychiatric inpatient admissions, etc.
  • Perform frequent home and/or other site visits to assess member needs and collaborate with resources as required.
  • Provide education to long-term care members and their families/caregivers on topics such as preventive care, procedures, healthcare provider instructions, treatment options, referrals, prescribed medication treatment regimens, and healthcare benefits.
  • Coordinate community resources, including medical, behavioral, and social services, to ensure members have access to necessary support.
  • Maintain accurate documentation and support the integrity of care management activities in our electronic care management system.
  • Comply with all policies and standards, including clinical guidelines and current state and federal guidelines.
Requirements
  • Master's degree in Mental Health or Social Work or Graduate from an Accredited School of Nursing and 2 – 4 years of related experience.
  • Licensed Behavioral Health Professional or RN with psychiatric experience based on state contract requirements.
  • Preferred experience in case management, patient advocacy, or coordinating community mental health and/or personal assistance or social services for members.
  • Strong clinical documentation skills and ability to utilize case management systems.
  • Proficient computer skills with Microsoft and Video Conferencing applications.
  • Ability to work independently with minimal supervision.
