Medical Search International is seeking a highly experienced Hospice Director of Clinical Services to lead a team of Interdisciplinary professionals in the Morris County Office.

Our client is dedicated to providing compassionate care, clinical excellence, and comfort to patients and their families facing the end of life. As the Hospice Director of Clinical Services, you will be responsible for coaching, influencing, and mentoring your team to strive for excellence in service and quality.

Key Responsibilities

  • Directly supervise up to 75 colleagues, including Nurses, Nurse Aides, Social Workers, Spiritual Care Providers, and other support functions.
  • Read and incorporate into practice the requirements of the Hospice Conditions of Participation for Medicare, applicable accreditation standards, and state or other rules and regulations.
  • Oversee the consultative process between the Medical Director and the Interdisciplinary Team, assist in requesting appropriate care orders, and attend to identified patient needs.
  • Attend and lead Interdisciplinary Team meetings, ensuring effective communication and collaboration.
  • Participate in the development and update of patient care policies and emergency procedures.
  • Ensure hospice services are available 24/7 and follow state regulatory requirements for supervision of nursing services.
  • Manage all members of the Interdisciplinary Team, including scheduling, productivity, mentoring, pay practices, time keeping, performance reviews, and colleague support.
  • Model desired behaviors, including timeliness, punctuality, attendance, collaboration, open communication, and equitable treatment.
  • Document in the patient's clinical record when required and demonstrate proficiency with documentation requirements.
  • Actively collaborate on QAPI activities and provide feedback and input into Performance Improvement.
  • Participate in performance improvement programs and promote hospice philosophy.
  • Uphold a personal philosophy of integrity and commitment and communicate proactively when schedule changes are needed.
  • Analyze and report on quality metrics, including Hospice Item Set, CAHPS, and symptom management outcomes.
  • Establish departmental goals consistent with corporate goals and objectives.
  • Supervise patient and family care as specified by the plan of care, assessing appropriateness, continuity, service, and quality of care.
  • Model and facilitate communication between colleagues, including medical records, business office, intake, clinical team, and leadership.
  • Participate in care delivery for patients as needed, including conducting admissions, recertification visits, routine visits, and discharge visits.
  • Manage the care for a caseload of patients if needed to support ebb and flow of census changes.
  • Participate in after-hours shifts as needed, including primary and back-up on-call.
  • Serve as the administrator on call when scheduled.
  • Make and/or delegate post-admissions calls.
  • Direct responsibility for the management of Pharmacy, Medical Supplies, Durable Medical Equipment, Mileage, and Labor for the program to meet budget expectations.
  • May be designated the administrator or alternate administrator for the licensure of the program based on state requirements.
  • Ensure maximum utilization of resources and promote a positive and collaborative culture.
  • This role is 80% leadership and 20% clinical, with a caseload as needed.

Requirements

  • NJ Registered Nurse license with a minimum of 3 years of healthcare leadership experience, preferably with Hospice experience.
  • BSN or higher required.
  • Experience with Electronic Medical Record systems a strong plus.
