Nurse Care Specialist

We are seeking a skilled and compassionate Nurse Care Specialist to join our team at Providence Mount St. Vincent. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality, resident-centered care to our patients.
Key Responsibilities- Develop and implement individualized care plans in collaboration with the interdisciplinary team.
- Administer medications and treatments as prescribed by physicians.
- Monitor and report changes in patient condition to the team.
- Provide education and support to patients and their families.
- Degree in nursing.
- Washington Registered Nurse License upon hire.
- National Provider BLS - American Heart Association within 30 days of hire.
- 1 year of LTC geriatric experience.
- Supervisory experience with nursing assistants (NAC's).
- Experience with an interdisciplinary team.
- 3 years of direct care experience.
- MDS/RAI experience.
