Job Title: Advanced Practice Provider - Nurse Practitioner or Physician Assistant

LiveWell Choice, a subsidiary of Longevity Health, is seeking an Advanced Practice Provider - Nurse Practitioner or Physician Assistant to join our team in Mt. Vernon, TX.

About the Role:

We are looking for a highly skilled and compassionate Advanced Practice Provider to provide high-quality care to our members in nursing homes. As an Advanced Practice Provider, you will be responsible for conducting comprehensive assessments, developing and delivering individualized plans of care, and collaborating with primary care teams to ensure the best possible outcomes for our members.

Key Responsibilities:
  • Provide collaborative primary care and services to a panel of Longevity Health members, including treating in the most appropriate setting based on the member's goals of care.
  • Execute and deliver Longevity Health Model of Care activities, treating in the most appropriate setting, and develop and implement advanced care planning.
  • Facilitate and administer Health Plan clinical operations functions, including prior authorization, Part A and B services, medication reviews, HEDIS and STARs quality gap closure, and promoting membership growth.
  • Serve as a trusted advisor to members, loved ones, and nursing home staff, and educate care teams on member needs and early identification of changes in condition.
  • Deliver effective and frequent communication and coordination with members' PCPs, loved ones, and care team members on members' care needs, goals of care, treatment plans, and changes.
Requirements:
  • State unrestricted licensed Advanced Practice Nurse or Physician's Assistant with National Board certification (AANP, ANCC, or AACN) as an Adult, Family, Geriatric, or Primary Care nurse practitioner or Physician Assistant.
  • Minimum of 6 months experience as a Nurse Practitioner or Physician's Assistant, with some experience in geriatric or palliative care.
  • Computer/Technology proficiency and excellent clinical skills, including assessment, diagnosis, treatment, and prescribing for the chronically ill population.
  • Ability to engage and communicate effectively with physicians, patients, family members, and nursing home staff.
