Hospice Care Specialist

At Southern Ohio Medical Center, we are seeking a compassionate and skilled Hospice Care Specialist to join our team. As a Hospice Care Specialist, you will provide personal care and support to patients and their families in a home setting. Your primary responsibility will be to assist patients with daily living activities, such as bathing, dressing, and grooming, while also providing emotional support and companionship.
Key Responsibilities:
• Provide personal care and support to patients in a home setting
• Assist patients with daily living activities, such as bathing, dressing, and grooming
• Provide emotional support and companionship to patients and their families
• Maintain accurate clinical records and documentation
• Participate in in-services, department meetings, and workshops to advance knowledge and skills
Requirements:
• High School Diploma or equivalent required
• Completion of approved nursing assistant, home care aide, or long term care nursing assistant training or equivalent experience required
• BLS certification within 90 days of hire required
• BMVL required (Must maintain eligibility to drive as determined by SOMC insurance carriers)
