Licensed Practical Nurse

As a Licensed Practical Nurse on our Inpatient Hospice team, you will provide compassionate and high-quality care to patients and their families. You will work under the supervision of the Nurse Manager and collaborate with the healthcare team to develop and implement individualized care plans.
Key Responsibilities
• Conduct thorough nursing assessments and document patient information accurately
• Implement nursing interventions and perform tasks as needed to meet patient needs
• Communicate effectively with physicians, RNs, and other team members to develop and update patient care plans
• Provide education and support to patients and families as needed
• Participate in hourly rounding and attend daily report, team meetings, and other related activities
Requirements
• High School Diploma or equivalent required
• Graduate of an accredited school of practical nursing required
• LPN (OH) licensure required, IV certification and BLS within 90 days of hire
• Approximately one year of experience as a LPN with experience working with terminally ill patients preferred
