About the Role:
  • Provide skilled nursing care and services to patients in their homes, utilizing the nursing process to assess, plan, implement, and evaluate patient care.
  • Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition, and collect, analyze, and interpret data and information from healthcare members.
  • Collaborate with the healthcare team to develop and implement individualized care plans that meet the unique needs of each patient.
Requirements:
  • Current licensure as an LPN/LVN in the state in which the LPN will practice.
  • Current TB or Chest X-Ray, and current BLS card.
  • One year prior professional nursing experience preferred.
