Maxim Healthcare Services is seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality care to patients in their own homes. As a LPN Homecare Specialist, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
About the Role:- Provide skilled nursing care and services to patients in their homes, utilizing the nursing process to assess, plan, implement, and evaluate patient care.
- Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition, and collect, analyze, and interpret data and information from healthcare members.
- Collaborate with the healthcare team to develop and implement individualized care plans that meet the unique needs of each patient.
- Current licensure as an LPN/LVN in the state in which the LPN will practice.
- Current TB or Chest X-Ray, and current BLS card.
- One year prior professional nursing experience preferred.
