Job Summary

Mercyhealth is seeking a highly skilled Social Worker MSW Case Manager to join our team. As a key member of our multidisciplinary team, you will play a critical role in assessing and addressing the complex psychosocial needs of our patients.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify patients' needs and develop individualized plans to address them.
  • Collaborate with patients, families, and healthcare teams to provide education, support, and counseling to promote healthy coping skills and lifestyle changes.
  • Develop and implement complex discharge plans to ensure patients receive the necessary resources and support to achieve optimal outcomes.
  • Act as a liaison between patients, families, and healthcare teams to facilitate communication and ensure seamless care transitions.
  • Provide crisis intervention services and identify and report child and elder abuse to local authorities as needed.
  • Stay up-to-date on community resources and payer/reimbursement practices to ensure patients receive the best possible care.
Requirements
  • Masters degree in Social Work from an accredited college or university.
  • Two years of related experience and/or training in an acute care setting.
  • Social Worker License in the state of practice required within six months of hire.
  • Case Management Certification (ACMA) preferred.

  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.
  • Proficiency in electronic medical records and other healthcare software.
